How to Register as a Patient

 

If you would like to register as a patient with our practice please ask at reception. We will require proof of your address and proof of identity. This can usually be done quite easily with a utility bill and photocard driving licence or passport.

 

We will check that your address is within our practice area (please see the map under Location / Practice Area or follow the link below) and you will then be asked to complete a registration form. We also ask that you complete a questionnaire giving us useful health information.

 

We operate a system of personalised lists, which means that you will register with a doctor whom you always see, except in the case of urgent treatment when your own GP is not available. This allows your doctor to get to know you well and we believe it improves the care that you receive.

 

When you register you can express your preference of doctor and we will try to accommodate your wishes, providing that is possible, given that the workload of new patients has to be shared between the doctors.

 

We have included a number of documents that you can download and print out if required. You will find these documents useful. Being familiar with the requirements and completing these in advance will help to speed up the registration process. We will also ask you to complete a local health authority registration form which is available from the Surgery (unfortunately we do not have this in an electronic format). 

 

It will help us deal with your registration more quickly if you are able to register between 2.00pm and 5.00pm. (We understand that this will not be possible if you need to register in order to see a doctor immediately.) The mornings are usually busy and this may make the process longer. In the afternoon staff are usually under less pressure and can spend more time helping you with the registration.

 

It will help speed up the registration process if you read the documents below. You may wish to complete the New Patient Questionnaire before you get to the surgery. Please note there are other forms which will need to be completed which are not included here.

Identification of carers

If you are a carer or have a carer it is useful for us to know. You may wish your carer to be able to receive your test results or be given access to other clinical information.

 

If you are a carer you are entitled to free influenza vaccination in the Autumn.
